In reply to Dr Morcos' comments on our recently published case report "Metastatic pulmonary calcification in renal failure: a new HRCT pattern", we do recognize tuberculosis as one of the infections that can cause multifocal pulmonary calcification. This possibility was considered in our case report, but both the clinical and radiological presentation, together with the negative Mantoux test (mentioned in the report), make it an unlikely cause for our patient's pulmonary calcification. Pleural calcification was not seen in our case, although it is interesting to note that it can happen in end-stage renal failure. Finally, the aim of our report was to introduce a new high resolution CT pattern for metastatic pulmonary calcification and, although Dr Morcos has enlightened us on other modalities that can detect pulmonary calcification, we feel that this discussion was beyond the scope of our case report.
