Book review |
This book forms another volume in the Springer-Verlag Medical Radiology series. An international set of authors provide a multidisciplinary account of anatomy, pathophysiology and imaging of portal hypertension. Medical, endoscopic and surgical treatment of portal hypertension are all covered briefly, with reference to data from randomized trials. Almost the whole of the second half of the book is devoted to various aspects of transjugular intrahepatic portosystemic shunts (TIPS).
The first part of the book includes a fascinating account of history extending back to the Eck fistula in 1877 (end-to-side portal caval shunt).
The imaging chapters are well illustrated with diagrams and radiographic studies, including good quality ultrasound and Doppler illustrations. CT and MRI are well illustrated, although the chapter on multislice CT would probably now be much longer. Quantification of flow and pressure are well covered.
The many chapters covering TIPS provide an excellent overview and include some experimental treatment not yet established, such as percutaneous retroperitoneal splenorenal shunt, which is described in pigs.
Of interest on the global scale, but likely very unusual in most first world departments, is a chapter on TIPS in Egyptian hepatic schistosomiasis.
There is a chapter dealing with covered stents for TIPS. There has been a lot of progress in this area during the last year or two and, as with much of the book, the references now seem a little out of date. Most of the chapters include references up to 1998 or 1999. Overall this is an excellent collection of articles dealing with aspects of portal hypertension and in particular its management by TIPS. Although within each chapter the authors have given an overview of their part, I wonder if the book would benefit from the final chapter or an introductory chapter giving a more complete overview of imaging diagnosis and treatment of portal hypertension.
