Radiotherapy treatment incident reporting has been a feature of this department for several years. Following an earlier paper published in the journalRadiography Today additional data have been gathered which have been combined with the original data. These data cover a period from January 1991 to December 1994 inclusive. Treatment incidents have been analysed with reference to the type, timing, possible reasons for the incidents and types of treatment unit. All new staff are briefed as to the incident reporting procedure and are requested to report any technical incident that results in incorrect treatment delivery.
