I read with interest the paper by Rupp et al [1]. I have, however, some concerns about their findings and conclusions.
End-to-end suturing was performed in only four of the operations, while the rest of the patients were treated by a variety of methods. The use of plantaris tendon grafts and of gastrocnemius flaps, alone or in combination, suggests to me that these patients were treated significantly later than the immediate post-injury phase. I have personally repaired 106 Achilles tendon ruptures, and was always able to suture the tendon stumps in an end-to-end fashion in all patients who were operated on within 72 h of the injury.
