The short communication by Tyrrell et al [1] regarding the use of a single view of the lumbar spine in GP referrals for backache highlights some of the ethical and professional difficulties encountered when attempting to control the use of an insensitive test in the setting of a common clinical problem. It is also another example of the one exception disproving the rule.
