Neal et al [1] are to be congratulated on the thoroughness of their review of tangential breast irradiation dosimetry. No radiation oncologist would disagree that current breast dosimetry is anything less than grossly imperfect. Most would also agree that it is questionable that improved dosimetry will improve cosmetic outcome. The latter is dependent not only on radiation dose, fractionation and volume, but also on surgical techniques and their sequelae. Therefore, one doubts that a "carefully controlled randomized controlled trial" is feasible.
