A 27-year-old male patient was referred to a rheumatology clinic because of mild pain in the neck of 3 years duration. The pain was mainly present in the morning and after a rest, associated with 10–20 min stiffness, subsiding with activity. He also noticed that he had had some limitation of neck motion. More recently, he complained of shoulder pain bilaterally upon raising his arms.
