A 76-year-old-man presented to the orthopaedic department with a 6-month history of pain in the left shoulder. The patient was otherwise well. A radiograph revealed an area of lucency adjacent to the greater tuberosity (Fig. 1). Radiographs of the cervical spine revealed spondylosis. Initial investigations including a chest radiograph and routine blood tests revealed no abnormality. A bone scan was performed which showed no evidence of increased uptake in the lesion (Fig. 2) or in the remainder of the skeleton.
