A comparison of conservative and radical surgery combined with radiotherapy in the treatment of stage 1 carcinoma of the breast

A retrospective study was made of 1,008 cases of stage I mammary carcinomas. The treatment of 415 was conservative surgery plus post-operative radiotherapy—of 593 radical mastectomy and radiotherapy. The five-year survival rate for the conservative surgery group was 79 per cent and the ten-year rate 71 per cent; the corresponding figures for the radical surgery group were 82 and 71·5 per cent. These differences were not statistically significant. A local recurrence developed in the spared mammary gland in 25·8 per cent of the patients managed by conservative surgery and was treated by new operation and radiotherapy. The radiotherapy given was with orthovoltage apparatus, with supervoltage apparatus it is possible to give a higher tumour dose and the results may be improved.
